About
Gold PTT is a topical solution of gold microparticles for photothermal therapy of acne and enlarged pores. Applied with low-frequency ultrasound, the particles settle in the pilosebaceous unit; a 1064 nm long-pulsed Nd:YAG then heats them so the overactive sebaceous gland is targeted selectively while the surrounding skin is spared. The manufacturer describes it as a medium that facilitates laser treatment, not a stand-alone medication.

How to use
Gold PTT in the treatment room, step by step.
- 01
Cleanse
Cleanse the face thoroughly. Protect the patient's eyes with gauze and goggles — the solution must not reach the eyes.
- 02
Apply with sonophoresis
Shake the vial and draw the solution into a syringe. Apply drop by drop, one area at a time, working it in with low-frequency ultrasound (40 kHz) for about ten minutes. The skin feels tacky when the particles have gone in.

Wipe the surface
Remove surplus solution gently with wet gauze. Hard rubbing pulls particles back out; skipping the wipe raises the risk of burns.

Laser
1064 nm long-pulsed Nd:YAG. Start at the manufacturer's standard setting (24 J/cm²), test-spot, and reduce to 20 then 10–15 J/cm² if the patient is uncomfortable. Serpentine passes with 20–30 % overlap, no stacking; about 10–12 minutes.

Aftercare
Cool compress if needed. No scrubs or bleaching creams for three days, avoid sun, SPF 30+. Dryness and light peeling for about a week is expected.

Course
Every two weeks, three to five sessions (every three weeks for sensitive skin, never more than four weeks apart). The manufacturer suggests a monthly maintenance session afterwards.

- Selective photothermolysis
- Gold absorbs the 1064 nm beam and converts it to heat where the particles sit — in and around the sebaceous gland — so the laser can run at lower energy than it would on skin alone. The manufacturer's target is a skin temperature of 40–42 °C.
- One laser
- The manufacturer recommends the 1064 nm long-pulsed Nd:YAG only. 755 nm alexandrite does not activate the gold; 800 nm diode and Q-switched Nd:YAG are not recommended. LED, IPL, RF and HIFU are not substitutes.
- Why two weeks
- Unlike ablative resurfacing, the tissue is only warmed, so the manufacturer sets a two-week interval and a course of three to five sessions. Visible change is expected from the third session.
- What to expect
- Dryness and peeling for about a week, transient redness and swelling, and in some patients a temporary flare before improvement. Residual particles leave the skin with the sebum.
- Rosacea
- Beyond acne and pores, the manufacturer and a published three-patient case series describe use in refractory papulopustular rosacea with the same laser.
- Delegation
- Cleansing, application and wiping can be delegated; the laser step belongs to the trained laser operator.

Cautions
- Allergy to metal or gold; allergy to alcohol
- Laser contraindications apply: active infection, connective-tissue disease, impaired wound healing, photosensitising medication, recent sun or tanning
- Wait 2–4 weeks after another laser treatment or peel in the same area
- Disclose metal implants in the treatment area
- Stop and seek medical advice on any adverse reaction
